Service description
Anyone who wishes to practice a healthcare profession on a self-employed basis, or who employs or intends to employ healthcare professionals, must notify the public health department with jurisdiction over the place of business of the start and end dates of such activity within one month (Section 12(1), sentence 1, HGöGD).
Upon commencing employment, the address of the branch office must be provided, and proof of authorization to practice the profession or use the professional title must be submitted.
Any changes regarding the required information must be reported to the public health department without delay (Section 12(1), sentence 2, HGöGD).

If employees hold different professional roles (e.g., at a medical care center), a separate report must be filed for each professional group.
Failure to register or deregister may result in a fine.
What documents are required?
- A copy of your professional license or medical license (notarization is not required)
- If applicable, field of specialization and documentation of subspecialty or additional certifications
What fees apply?
The ad is free of charge.
A written confirmation of the filing of a report pursuant to § 12 HGöGD is subject to a fee. In accordance with Section 6251 of the Administrative Cost Regulations for the Ministry of Social Affairs and Integration (VwKostO-HMSI), the fee is currently 15 euros.
Legal basis
§ 21(2) HGöGD
"Any person who, intentionally or through negligence, fails to report the independent practice of a healthcare profession or the employment of healthcare professionals within one month of commencing such activities is committing an administrative offense. This administrative offense is punishable by a fine of up to 3,000 euros."
