Service description
If you hold a firearms permit and have lost it, you may either waive it, provided that no firearms subject to licensing are listed on it, or apply for a apply for a replacement document.
Waiver of a firearms permit
If No firearms subject to licensing firearms on the firearms license document (e.g., on a blank yellow Wfirearms ownership card or a limited firearms permit), refer to the lost permit document . To do so, the Declaration of Loss.pdf and the waiver form must be completedRequesting a Replacement Document
If you would like to request a replacement document, please submit the appropriate application form .
